Post‑seg, run a connected‑components pass and keep only the largest island attached to the jaws — e.g., 3D Slicer Segment Editor > Islands: Keep largest island / Remove small islands (Segment editor — 3D Slicer documentation); it usually pops the “cotton ridge” without HU tweaks, but set the size cutoff low enough to spare thin plates. Which app are you using?
Tiny erosion (1–2 voxels) on the bone mask breaks the cotton’s skinny bridge, then I split/delete that island and dilate back — keeps the cortex intact without cranking thresholds; just go easy around thin plates. What are you working in, @OP?
